Eligibility:
a. The Care Discount Program (“the Program”) is open to all registered Assisted Living Facilities, Independent Living
Communities, and Care Homes.
b. The Program is not available to individuals or non-related organizations.
Enrollment:
a. Organizations interested in participating in the Program must complete the enrollment process.
b. The enrollment process may require the submission of relevant documents, such as proof of registration as an
eligible organization.
Discount Offer:
a. Participating organizations will be eligible for a specified discount on services offered by myLife A.D.
b. The discount offer may vary and is subject to change without prior notice.
c. The discount cannot be combined with any other ongoing promotions or offers unless explicitly stated by myLife A.D.
Terms of Use:
a. Participating organizations agree to use the discount solely for their own organizational needs and not for personal
or resale purposes.
b. The discount provided through the Program is non-transferable and cannot be shared or sold to third parties.
Program Modifications:
a. myLife A.D. reserves the right to modify or terminate the Program at any time without prior notice.
b. Changes to the Program, including changes to discount rate, will be communicated to the contact lead through
email.
Liability:
a. Participating organizations acknowledge that the Program is provided as a courtesy and that myLife A.D. is not liable
for any direct or indirect damages or losses incurred through participation in the Program.
b. myLife A.D. does not provide any warranty or guarantee for the services offered under the Program.
Confidentiality ability:
a. Participating organizations agree to maintain the confidentiality of any discount codes, or other information related to the Program that they may have access to.
Governing Law:
a. These terms and conditions shall be governed by and construed in accordance with the laws of the jurisdiction in
which the organization is located.
Agreement Acceptance:
a. By enrolling in the Program, participating organizations agree to abide by these terms and conditionstions.
